Too much of this vitamin may raise heart disease risk

A study published in Nature Medicine has stirred debate in cardiometabolic circles.

It suggests that excess levels of niacin (vitamin B3)—long considered benign at nutritional levels and historically used in pharmacologic doses to influence lipids—are associated with an increased risk of major cardiovascular events such as heart attack and stroke.[]

The study's findings

Researchers led by Stanley Hazen, MD, PhD, of the Cleveland Clinic, used untargeted metabolomics to identify markers in fasting plasma that predicted adverse cardiac events.

They found that elevated levels of niacin breakdown products, particularly N1-methyl-4-pyridone-3-carboxamide (4PY), were associated with a significantly higher risk of cardiovascular events over approximately 3 years, even after adjustment for traditional risk factors.

In validation cohorts from both the US and Europe, individuals in the highest quartiles of these metabolites had roughly 1.6–2 times greater risk of major adverse cardiac events compared with those in the lowest quartiles.

Mechanistic studies in mice suggest that 4PY directly promotes vascular inflammation, increasing expression of a protein called VCAM-1 and facilitating leukocyte adherence to the endothelium.[]

Importantly, the investigators also noted that about 25% of Americans had elevated 4PY levels, likely from fortified foods and supplements, raising questions about the upper limits of safe intake.[]

Clinical context and nuances

From a clinical standpoint, these findings both echo and extend prior paradoxes in niacin research:

  • Pharmacologic niacin has long lowered LDL and raised HDL, but large randomized trials failed to reduce the risk of cardiovascular disease among those who take statins.[]

  • This new work suggests a novel inflammatory pathway that might partly explain why lipid changes did not translate into expected outcomes.

However, there are several critical caveats to consider:

  • This was an observational association, not a randomized intervention; causation cannot be established.

  • Dietary intake, supplement use, and other confounders were not directly measured.

  • Biomarkers such as 4PY are not yet validated clinical assays, and normative ranges for risk stratification do not exist.

Finally, other epidemiologic data still suggest that routine dietary niacin intake within recommended ranges is associated with neutral or even favorable outcomes in some cohorts. []

“The main takeaway is not that we should cut out our entire intake of niacin—that’s not a realistic or healthy approach. Niacin is a vitamin and essential to our health. But an excess amount appears to be a concern," Dr. Hazen told Health.[]

What should you tell patients?

For clinicians, the key takeaways are both practical and protective:

1. Routine supplement use of high-dose niacin should be reevaluated. Unless there is a specific, evidence-based indication (eg, certain dyslipidemias not controlled by other therapies), advise patients that high-dose niacin supplements may not be beneficial—and could be harmful.

2. Ask about fortified foods and multivitamins. Patients may unknowingly ingest significant amounts of niacin through fortified cereals, breads, and supplements. Clarify total intake when counseling on nutrition and supplements.

3. Don't overinterpret these data as proof of causality. This study suggests a mechanism and hypothesis that warrants further clinical and mechanistic investigation, but it should not result in abrupt changes to established dietary recommendations.

4. Consider cardiovascular risk holistically. As with many micronutrients, context matters: Dietary sources at recommended dietary allowance levels are unlikely to be harmful, whereas therapeutic or excessive intake without medical supervision carries potential risk.
