The Patient

Trinity Peterson-Mayes, 24, of Arizona, is a two-time cancer survivor. At 2 months old, she was diagnosed with a rare childhood cancer, and when she was 11, she was diagnosed with an aggressive bone cancer. []

In February 2026, she attended a gathering with friends and consumed a homemade fermented swordfish dish. By her own account, the food “tasted horrible,” but she initially expected nothing more than mild GI upset. []

Within days, her clinical trajectory shifted dramatically.

The Tests

Her initial symptom—progressive dysphagia—was subtle but ominous. She described going from difficulty drinking liquids to being unable to swallow even water within roughly 24 hours. []

She went to a local hospital, where no clear diagnosis was established, and she was almost sent home. But she insisted that something was wrong, and her symptoms escalated—including choking episodes, bulbar dysfunction, and progressive weakness—and she was transferred to a tertiary care center for neurological evaluation.

The Diagnosis

Peterson-Mayes was ultimately diagnosed with foodborne botulism, caused by Clostridium botulinum toxin exposure—likely from improperly fermented fish. Botulinum toxin blocks presynaptic acetylcholine release at the neuromuscular junction, producing a descending flaccid paralysis. []

Clinically, this manifested in her case as:

  • Dysphagia and dysarthria

  • Progressive generalized weakness

  • Loss of motor function, including inability to speak or move

At its peak, she was completely paralyzed and required airway support. Two additional individuals who consumed the same meal also developed botulism, though with less severe courses. []

“Most emergency physicians go their whole career without seeing a case,” Frank LoVecchio, DO, an emergency medicine physician in Arizona, told KBNX. “It causes this paralysis of your muscles, and most important being, you know, your chest muscles, those that are responsible for breathing,” he explained. “In the United States, there's about two dozen cases per year on average of food botulism.” []

The Treatment

Management required ICU-level care, including:

  • Intubation and mechanical ventilation

  • Central venous access and enteral feeding support

  • Administration of botulinum antitoxin 

The antitoxin halts progression of paralysis but does not reverse established deficits. [] Her recovery depends on regeneration of neuromuscular junction function, which can take weeks to months. []

She now faces prolonged rehabilitation to regain basic functions such as speaking, swallowing, and ambulation. []
