Wagner S et al. – Laparoscopic dismembered pyeloplasty, if performed by expert surgeons in high–volume centres, presents results that are comparable with open surgery, with a lower surgical trauma for the patients.

Exclusive Author Commentary
Francesco Greco, 10/30/09

Laparoscopic pyeloplasty is a minimally invasive alternative in the treatment of UPJ obstruction and was developed in the early 1990s. After almost 20 years this technique represents a hard thema of discussion about its feasibility and safety. Moreover the technical difficulties associated with this surgical procedure have limited its diffusion among the urologists. In our manuscript 105 patients underwent a laparoscopic dismembered pyeloplasty for ureteropelvic junction obstruction. All surgical procedures were performed by two surgeons (P.F., F.G.) who had completed at least 40 LDP each before the beginning of the study, thus reducing the learning-curve effect. After 5 year-follow, the successful rate was 96.2%, conferming that laparoscopic pyeloplasty, if performed by expert surgeons in highvolume centres, presents results that are comparable with open surgery, with a lower surgical trauma for the patients.
