Marickar YMF et al. – Open surgery still remains the sheet anchor of treatment of urinary stones in many patients in Kerala. Newer lesser invasive procedures should be ethically selected. Decisions should be patient based, taking into consideration the economic feasibility for the procedure proposed.

Y. M. Fazil Marickar, 10/23/09

Unfortunately many patients with medium sized ureteric stones are rushed to the theatre for uretero renoscopy. This is an expensive modality of treatment and in many cases avoidable. Similarly ESWL and PCNL are offered to many patients with small non obstructing calyceal stones. Failure rates are very high. Surgery becomes mandatory in many large stones due to non availability of equipment, expertise and financial capability of the patient. Thus, in many Government hospitals in the region, surgery becomes the commonest mode of stone retrieval. Judicious decision making is recommended.
