Predictors of clinical metastasis in prostate cancer patients receiving androgen deprivation therapy

Abouassaly R et al. - In a trial to identify predictors of clinical disease progression in pts with prostate cancer (PCa) who were receiving androgen deprivation therapy (ADT), it was shown that younger men with high-risk disease appear to have worse prognosis than older men with similar disease. This, along with other prognostic variables, may help identify candidates for clinical trials evaluating secondary treatments for pts with castrate-resistant disease.
Methods- Of 13,740 men with biopsy-proven PCa who were enrolled, 4003 men treated with ADT after diagnosis without evidence of metastases at treatment initiation were identified.
- Primary endpoint was development of bone metastasis.
- Clinical and pathologic characteristics were compared between pts who developed metastasis and those who did not using chi-square tests in a Cox proportional hazards regression model.
- Mean age of the men was 70 yrs.
- 191 men (4.8%) progressed to metastatic disease at a median of 18 mos from initiation of ADT.
- On multivariate analyses, risk category, percent of biopsies positive >33%, age ≤65 yrs at diagnosis, and prostate-specific antigen velocity on ADT were found to be significantly associated with development of metastatic disease after ADT.
