Ornellas AA et al. - The authors report the clinicopathologic data on 3 additional cases of sebaceous carcinoma arising in the penis. Treatment is debatable in view of the fact that this kind of tumor has a high recurrence rate and early regional lymph node involvement. The authors used preoperative lymphoscintigraphy, intraoperative lymph node mapping and sentinel node biopsy before performing a bilateral inguinal lymphadenectomy in 1 of 3 patients treated.

| Only 2 cases of sebaceous carcinoma had been reported on the penis and our series adds 3 additional cases.We discussed in a straighter manner the therapy advices for sebaceous carcinoma of penis.The exact incidence of metastasis is uncertain due to the small number of cases that have been reported.One of our patients with clinically bilateral negative inguinal lymph nodes underwent systematic bilateral inguinal lymphadenectomy after lymphoscintigraphy with identification of sentinel lymph nodes.We discussed the role of dynamic sentinel node biopsy, showing that this technique has been associated with a false-negative rate of 16% (Kroon et al.) and justifying why we performed bilateral lymphadenectomy in this case.Sebaceous carcinoma of penis is a very aggressive tumor with early lymph node involvement and early-disseminated metastases |
