We used the SEER-Medicare linked database to estimate mean per-patient costs related to prostate cancer (PCa) during the first year following diagnosis. Cost estimates and treatment patterns are presented by initial cancer stage. We found that almost one-third of all PCa patients did not receive an active therapy. These patients were slightly older (mean age: 76 years) vs. those receiving active treatment (mean age: 73.2 years). Receipt of external beam radiation and brachytherapy was more common among early vs higher staged patients. Very few patients received cryotherapy (0.1%) or bilateral orchidectomy (0.2%). We also found that PCa-related costs increased according to cancer stage (Stage I: $11,590; Stage II: $12,191; Stage III: $13,920; and Stage IV: $18,371 All: $13,091). Our analyses shed light on the magnitude of first-year PCa-related costs and may be useful in estimating the potential economic benefits of prevention strategies. Future research should focus on estimating costs during later phases of care (i.e., continuing and terminal care) and producing life-time estimates.
