Prospective study of factors predicting adherence to medical advice in men with testicular cancer
Journal of Clinical Oncology, 05/04/09
Moynihan C et al. – Study reports that patients who perceived an unsatisfactory affective relationship with their clinician including inability to trust him and that they were not being treated as "a person" were more likely to disregard medical advice regarding follow-up. Therefore the attention to the ways young men may wish to communicate with their clinicians is important.
Methods- Aim was to identify predictive factors of adherence to medical advice in pts with newly diagnosed testicular cancer
- A prospective study measured:
- aspects of the doctor–patient interview
- a range of demographic, psychological, social, and medical factors, and
- attendance behavior on f/u
- 184/209 new pts consented and completed questionnaires
- The nonadherence end point: 2 failures to attend an outpatient appointment at least 1 month apart, despite a written reminder
- 17% were classified as nonattenders
- No differences between attenders and nonattenders in psychosocial and medical variables that could predict nonadherence to medical advice
- An association between nonattendance and a pt's perception of an unsatisfactory affective relationship with his clinician was observed
