Gul W et al. - Because of the widespread use of body imaging techniques, there is increasing documentation of adrenal gland involvement with metastasis. It now is the fourth most common site of metastasis, after lung, liver, and bone. The tumors that are most prone to spread to the adrenal are melanoma, breast, lung, and various GI cancers, most commonly stomach. Adrenal metastases are almost always clinically silent, but with extensive and bilateral metastasis, hypoadrenalism may occur. Such Addisonian presentations often exhibit only vague GI symptoms, a hint of their cause only being found in the accompanying classic pattern of electrolyte disorders: hyperkalemia and hyponatremia.
