Gilbert S et al. – Diagnostic performance data support the clinical usefulness of EBUS in the evaluation of patients with a radiographically abnormal mediastinum. It should be considered complementary to mediastinoscopy rather than substitutive.

All patients in our review had at least one mediastinal lymph node station that was abnormal on CT and/or CT/PET scan. The use of EBUS to obtain FNA of mediastinal lymph nodes is likely to decrease the number of surgical procedures (i.e. mediastinoscopy) needed. When mediastinal lymphadenopathy is due to malignant disease, the consequences of a false-negative diagnostic test may be serious (e.g.: suboptimal timing of surgical resection, delay in the delivery of definitive chemoradiation). Therefore, the false-negative rate is a key attribute of any diagnostic test used to determine lymph node status in the mediastinum. There is some overlap in the published false-negative rates of mediastinoscopy (1.6-8.8%) and EBUS (6.2-13%). Based on our data, these two approaches certainly appear to be complementary. Close collaboration between radiologists, pulmonologists, medical oncologists and thoracic surgeons remains of the essence in the management of these patients.
