A new concept for individualised pain treatment in a surgical unit

Schmelling W – The concept is easy to use and time reducing. The monitoring scheme gives an overview of treatment effects and makes it possible to individualize pain treatment, so that patients treated for acute and postoperative pain, achieves satisfactory pain relief.
Methods- Development and implantation of a concept for pain treatment including:
- An educational instruction containing aim of treatment
- Two evidence–based treatment regimes due to American Society of Anesthesiologists (ASA) classification
- Measurement of pain routinely and as needed using a validated method
- Pain score based opioid dosing with no upper limit
- Delegated prescription right to DOS staff nurses
- Educational monitoring scheme which as well serves as medical record
- Scoring of pain, prescription and administration of pain medicine
- Laminated pocket size copy of the concept to relevant staff members
- Short introduction to the concept for doctors and nurses
- Inventory concerning doctors and nurses satisfaction with the concept
- Concept in use for 18 months
- Audit of medical record, concerning the effect of pain treatment 16 to 36 hours after surgery
- The inventory showed great satisfaction with the instructions, overview, use of time and the pocket size copy of the concept.
- The medical record audit demonstrated that the patients achieved a high degree of satisfactory pain relief.
