Pancreas allograft thrombosis following intravenous immunoglobulin administration to treat parvovirus B19 infection

Muthusamy ASR et al. - This case report describes a patient who developed graft thrombosis after intravenous immunoglobulin administration to treat acute parvovirus B19 infection. The potential role of hypercoagulability in graft thrombosis and the implications for immunoglobulin therapy in transplant patients with hypercoagulable states is discussed.

| Pancreas allograft thrombosis is a common problem with pancreas transplantation, and is related to hypercoagulability. We routinely manage this problem with thromboelastography directed anticoagulation, reducing the risk of graft thrombosis. The intravenous immunoglobulin administration probably resulted in a temporary hypercoagulable state resulting in graft thrombosis within 24 hours of administration. This report is to encourage clinicians to employ appropriate prophylactic anticoagulant measures in patients with hypercoagulability receiving intravenous immunoglobulin. |
