Greco KA et al. - The outcomes of RARP in elderly men are largely comparable to those in younger men, with the exception of higher pathological Gleason grade, a transient delay in return of continence, and taking longer to return to driving after surgery. Advanced chronological age should not be a contraindication for RARP in patients with clinically localized prostate cancer, but expectations should be managed preoperatively.

As the life expectancy of men reaches well in the 80’s, those that are affected by clinically significant prostate cancer will likely rise. Currently, the age at which to stop prostate cancer screening remains controversial and many physicians may stop screening due to the morbidity associated with treatment of prostate cancer. Our goal in this study was to determine the effect of surgical treatment of prostate cancer (robotic prostatectomy) on aged men. Remarkably, men aged greater than 70 years have similar outcomes to younger men. Men aged > 70 yrs had a greater hospital stay (2.8 vs. 1.8 days, p=0.02) and greater pad usage at 6 months that recovered by 12 months after surgery. We discuss these findings with aged men prior to surgery and counsel them preoperatively regarding pelvic floor strengthening and rehabilitation prior to surgery.
