The role of sentinel node biopsy in ductal carcinoma in situ of the breast
European Journal of Surgical Oncology, 09/08/08
Polom K et al. – Sentinel node biopsy (SNB) for diagnosis in ductal carcinoma in situ (DCIS) remains controversial due to few cases of axillary lymph node metastases; the related clinical challenge is a primary diagnosis of DCIS with invasive components on mammotomic biopsy
Methods- Study of the diagnostic value of sentinel nodes in female pts with primary DCIS using core needle stereotactic biopsy
- Core needle biopsy for diagnosis of DCIS in 216 pts
- SNB to determine lymph node involvement for 183 pts
- Axillary lymphadenectomy for pts with metastases to the sentinel node
- Of 183 pts, 10 (5.5%) had metastases to sentinel lymph node
- Of these 10 pts, invasive ductal carcinoma in 6 cases (3.5%); 1 case (0.5%) of invasive lobular carcinoma
- DCIS with metastases to sentinel lymph nodes as final diagnosis for 3 pts (1.5%): 2 DCIS; 1 DCIS with microinvasion
- Axillary lymphadenectomy for pts with abnormal SNB showed other axillary lymph node involvement in 4 pts
