Epidural anesthesia reduces post-CABG pain

As published in the Journal of Cardiac Surgery, thoracic epidural anesthesia (TEA) plus general anesthesia (GA) is superior to GA alone in managing post-operative CABG pain. Forty patients undergoing elective CABG were randomized to receive TEA (0.25% bipivacaine [10-20 mg/h]) intra-operatively and for 24 h post-operatively plus GA or GA alone. Post-operative pain at rest and with coughing was significantly less for up to 1 month post-operatively in the TEA + GA group compared to the GA alone group. Furthermore, analgesic use, mechanical ventilation time, ICU stay time, and hospital stay time were reduced in the TEA + GA group compared to the GA alone group.

NT-proBNP predicts outcome following emergent surgery

As published in the British Journal of Anaesthesia, measurement of N-terminal pro B-type natruiretic peptide (NT-proBNP) pre- and post-operatively predicts outcome in patients > 50 years of age (n=297) undergoing emergent non-cardiac surgery. A pre-operative NT-proBNP > 725 pg/ml was associated with a 4.8-fold RR of non-fatal MI, acute heart failure, or death between surgery and the 3-year post-operative follow up. Similarly, a post-operative NT-proBNP > 1600 pg/ml was associated with a 4.0-fold RR of non-fatal MI, acute heart failure, or death between surgery and the 3-year post-operative follow up. A pre-operative NT-proBNP > 1740 pg/ml was associated with a 6.9 RR for major adverse cardiac events during the post-operative hospital stay.

Increased risk of skin cancer in RCC treated with multikinase inhibitors

As published in Dermatologic Surgery, RCC patients (n=69) treated with multikinase inhibitors are at increased risk for squamous cell carcinomas and basal cell carcinomas. After a median treatment of 13.5 months, 13.5% and 6.3% of patients treated with sorafenib and sunitinib developed skin cancer, respectively.
