Destructive arthritis in Behçets disease: A report of eight cases and literature review

Frikha F et al. – Study concludes that despite the rarity of destructive arthropathies in Behcet's disease (BD), this unusual form deserves to be known and should be considered in the differential diagnosis of seronegative destructive peripheral joint disease. When osteo-articular involvement seems to be predominating in patients with BD, follow-up should involve regular assessment of articular consequences of the disease, including radiographic investigation.
Methods- Aim was to define the epidemiological, clinical and radiological features of osteoarticular manifestation of BD
- Medical records of 553 pts with Behçet's disease seen over 25-year period were reviewed
- All pts fulfilled The International Study Group of Behçet's Disease criteria
- Pts with destructive arthritis defined by radiological changes were included
- Rheumatologic manifestations were observed in 71.1% pts
- 8 pts (1.4% overall, 2% among pts with rheumatologic manifestations) had presented with destructive arthritis
- Joint symptoms involved:
- Knee in 2 cases
- Wrist in 1 case
- Elbow in 1 case
- Sternoclavicular joint in 2 cases
- Foot in 1 case, and
- Tarsal scaphoïd in 1 case
- There was recurrent arthritis at the same joint in the majority of cases
- X-ray examinations revealed radiological changes:
- Global narrowing of the joint in 1 case (knee)
- Narrowing of the joint with geodes in 3 cases (knee, sternoclavicular)
- Isolated geodes in 2 cases (tarsal scaphoid, foot), and
- Severe lesions with ankylosis in 2 cases (two elbows, right wrist)
- Joint manifestations are common in pts with BD, but destructive arthritis is rare
