What can we learn from the presence of anti-cyclic citrullinated peptide antibodies in systemic lupus erythematosus?

Zhao Y et al. – Study demonstrates that anti-CCP antibodies have a frequency of 13.8% in Chinese systemic lupus erythematosus (SLE) patients and its presence is closely associated with the onset of arthritis and bone erosion.
Methods- An investigation of the prevalence and clinical significance of anti-CCP Abs in Chinese SLE pts
- Serum samples from 138 SLE pts were examined for anti-CCP
- Associations of anti-CCP with clinical and laboratory features, especially arthritis, were analyzed
- Prevalence of anti-CCP was 13.8%
- 70/138 SLE pts had experienced arthritis, of whom 14 pts were anti-CCP+
- Anti-CCP Abs were more frequently found in SLE pts with arthritis vs those w/o
- A correlation between anti-CCP and rheumatoid factor (RF) was found in SLE pts with arthritis
- Frequency of arthritis was higher in SLE pts with anti-CCP vs those w/o
- 8/138 SLE pts showed joint erosions on radiographs
- When compared with anti-CCP- pts, erosive arthritis occurred more often in anti-CCP+ pts
- 2 pts w/o anti-CCP and RF who had erosive arthritis were anti-RA33 Abs positive
- All of 8 SLE pts with erosive arthritis fulfilled 1987 ACR criteria for RA
- With regard to other clinical and laboratory features, no differences between SLE pts with arthritis and w/o or between anti-CCP+ pts and anti-CCP- pts
