Injectable hyaluronan for the treatment of carpometacarpal osteoarthritis: Open label pilot trial

Mandl LA et al. – Study demonstrates that intra-articular Hylan G-F 20 injections reduced pain and improved function in patients with carpometacarpal osteoarthritis (CMC OA) at 26 weeks. Larger studies are needed to confirm these results, and to determine predictors of response to treatment.
Methods- Aim was to determine the efficacy of inra-articular Hylan G-F 20 in pts with CMC OA
- 32 pts with CMC OA were injected with Hylan G-F 20, once weekly for 3 consecutive wks
- Pts assessed at 4, 12, 20 and 26 wks after the first injection
- A last-value carried forward analysis was performed
- Average age was 64 yrs; 69% were female and 97% Caucasian
- 53% had at least one previous corticosteroid injection in the affected CMC joint
- At 26 wks, mean VAS for pain had improved significantly
- Disabilities of the arm, shoulder and hand questionnaire (DASH) scores also improved
- Neither key strength nor opposition grip strength improved
- VAS scores for pain at 26 wks showed good correlation with patient satisfaction
- AEs related to the injections: 3 episodes of post-injection pain and swelling, and 1 case of crystal proven pseudogout
