Shoulder arthritis and the young patient
Current Orthopaedic Practice , 07/28/09
McCarron JA et al. – Article discusses the options for the management of young patients with glenohumeral arthritis.
- The suspected causative risk factors for chondrolysis, an iatrogenic cause of arthritis, should be minimized or avoided whenever possible
- The first line of treatment for arthritis: should be nonsurgical, followed by less invasive, joint preserving surgical options
- When these fail, hemiarthroplasty, with or w/o biologic glenoid resurfacing or total shoulder arthroplasties are effective at reducing pain and improving function
- However, these options are associated with deteriorating function, pain, and glenoid-sided problems at longer time points
- Treatment of glenohumeral arthritis in younger pts must be:
- Highly individualized, and
- Based on a detailed discussion with the patient regarding their expectations and functional requirements
- A multi-disciplinary approach that delays more aggressive procedures in favor of a long-term outlook while minimizing pain should be adopted
