Anti-U11/U12 RNP antibodies in systemic sclerosis: A new serologic marker associated with pulmonary fibrosis

Fertig N et al. – Study reports that anti-U11/U12 RNP antibodies were present in the sera of approximately 3% of patients with systemic sclerosis (SSc) and were a marker for lung fibrosis, which is often severe.
Methods- Identification of clinical features associated with a new serum autoantibody against U11/U12 RNP in SSc pts
- Antibody specificity and prevelance was confirmed by immunoprecipitation, RT-PCR, and Southern blotting
- Anti-U11/U12 RNP autoantibody-positive and negative SSc pts were compared based on demographic, disease classification, clinical variables, and survival
- 33 pts with anti-U11/U12 RNP antibodies were identified
- In 2 consecutive series of SSc pts first seen at 10-yr intervals (1994-1995 and 2004-2005), prevalence of antibody-positive pts was 3.2%; 52% of these had limited cutaneous involvement
- All pts had Raynaud's phenomenon and 82% had GI involvement
- None had intrinsic pulmonary arterial hypertension
- Most significant clinical difference between Ab-positive and negative cohorts was the prevalence of lung fibrosis (79%) in positive pts
- GI involvement was also significantly increased in Ab-positive group
- Pts with RNP Ab and pulmonary fibrosis had a 2.25-fold greater risk of death than negative pts with pulmonary fibrosis
