Carter J et al. - Reactive arthritis (ReA) is an inflammatory arthritis that arises after certain types of gastrointestinal or genitourinary infections, representing the classic interplay of host and environment. This insight into disease initiation has led to significant advances in the pathophysiology of this condition.

HLA-B27 is believed important in determining disease susceptibility, yet recent data downplay its importance and suggest it might be a better predictor of disease severity. Although bacteria are known to trigger ReA, the role of antibiotics remains ill defined. Anti-TNF therapy has proved efficacious in the other SpAs, but sufficient data are lacking and theoretic concerns with their use remain. Pathophysiology that surrounds disease initiation needs to be targeted in hopes of finding a definitive treatment.
