Schafranski MD et al. – Study describes a case of a Chlamydia trachomatis-related reactive arthritis refractory to methotrexate and sulphasalazine that was successfully treated with the monoclonal antibody anti-TNF-α and infliximab. Several pathogens have been linked to the development of reactive arthritis, however no guidelines regarding its treatment are available. Higher serum TNF-α levels in pts with chronic reactive arthritis are reported when compared with pts with the acute form of the disease, suggesting that this pro-inflammatory cytokine may take part in the progress to the chronic form of the disease. This is the first case of effective treatment in a Chlamydia-related situation. Given the response reported here, larger controlled trials are needed to better evaluate the role of these agents.
