Cartilage homeostasis in health and rheumatic diseases

Goldring MB et al. – This review discusses the current knowledge about the specific cellular and biochemical mechanisms that regulate cartilage homeostasis and pathology.
- In RA, mediators involved in immunomodulation and synovial cell function, including cytokines, chemokines, and adhesion molecules, have primary roles in the inflammatory and catabolic processes in the joint
- They may also, directly or indirectly, promote cartilage damage
- Despite all this our increasing knowledge, the development of disease-modifying therapies for OA pts has been elusive
- In RA, there is still a need for therapeutic strategies that prevent the extensive cartilage and bone loss, despite the clinical success of anti-TNF therapy for RA
- Further work using the principles of cell and molecular biology, such as those described in this review, will be necessary for uncovering new therapies for targeting cartilage destruction in both degenerative and inflammatory joint disease
