Thrombocytopenia as a presenting feature of Kawasaki disease: A case series from North India

Singh S et al. - Article describes six children with Kawasaki disease (KD) who had thrombocytopenia at disease onset. This is an unusual finding and has clinical implications in the diagnosis of this condition.
- Like most other vasculitic disorders, the diagnosis of KD is based on clinical features; no laboratory test pathognomonic of the condition is reported
- Serum levels of proinflammatory cytokines including IL-6, TNF-α have been shown to be elevated in pts with KD
- High levels of IL-6 results in fever and thrombocytosis in conjunction with thrombopoeitin in response to tissue injury
- Thrombocytopenia is an unusual finding in KD and has been infrequently reported in the literature
- In this study 3 of 6 cases of KD with thrombocytopenia developed coronary artery dilatation which was subsequently resolved on follow up
- Thrombocytopenia may, therefore, well be a risk factor for coronary artery abnormalities (CAA) during the acute phase of KD
