Treatment of sicca symptoms with hydroxychloroquine in patients with Sjögren's syndrome

Rihl M et al. – Study demonstrates that hydroxychloroquine (HCQ) treatment led to a beneficial effect on xerostomia in patients with primary Sjögren's syndrome (pSS) who lack severe organ manifestations. The response was greater in α-fodrin-positive patients.

Methods

Study to evaluate the efficacy of HCQ for glandular function, i.e. saliva and tear production in pSS pts
Group A (n=14): pts were anti-Ro and/or -La Ab positive except one; were treated with HCQ for up to 6 mo
Group B (n=21): control group with objective sicca symptoms and positive α-fodrin Ab
Glandular function was determined by Saxon's and Schirmer's tests for the dominant eye at baseline and at the end of the treatment

Results

Group A: an increase in saliva production after HCQ treatment
Particularly the α-fodrin-positive pts responded to HCQ
Group B: an increase in tear production
A positive correlation between the α-fodrin IgA Ab concn and the Schirmer's test at baseline and after treatment in this group
