Cademartiri F et al. – CT–CA proves high diagnostic performance in patients with or without symptoms and with zero CACS. The prevalence of significant disease detected by CT–CA was not negligible in asymptomatic patients. The role of CT–CA in asymptomatic patients remains uncertain.

Stratification of cardiovascular risk is a very tough task. Several methods and investigation have been used, mainly epidemiology based. However, the individualization of cardiovascular risk is the most important thing to consider. We cannot treat (medically or interventionally)$ all patients with a certain risk profile, and we have to aknowledge the cost of treating hunderd-thousand patients to reduce one single event in the population. CTCA can play a major in stratifying patients and individuals with no coronary artery disease, with non obstructive disease and with obstructive disease. CTCA is the only non invasive tool able to reliably do so, at the moment.
