Adherence to long-term surveillance mammography among women with ductal carcinoma in situ treated with breast-conserving surgery

Nekhlyudov L et al. - In a study to determine whether mammographic surveillance after breast-conserving surgery (BCS) occurs and by whom, it was concluded that surveillance mammography after BCS among insured women with ductal carcinoma in situ (DCIS) often did not occur yearly and declined over time after treatment. Pts and providers must remain vigilant about surveillance after BCS.
Methods- Medical records were received of women diagnosed with DCIS and treated with BCS.
- Using descriptive statistics, generalized estimating, and logistic regression modeling, rates and predictors of surveillance mammography were examined over a 10-yr period after BCS.
- 3037 women were observed for a median of 4.8 yrs.
- Of 2676 women observed for at least 1 yr after BCS, most (79%) had at least 1 surveillance mammogram during the first yr of follow-up; 69% in yr 5 and 61% in yr 10.
- Among those observed for 5 yrs, surveillance mammograms were more likely among women age 60 to 69 yrs, users of menopausal hormone therapy at diagnosis, as well as those treated with adjuvant radiation and adjuvant radiation with tamoxifen.
- Surveillance mammograms were less likely among obese women.
- Findings were similar among women observed for 10 yrs.
- Only 34% and 15% of women observed for 5 and 10 yrs, respectively, had a surveillance mammogram during each yr of follow-up.
