Identifying left ventricular dysfunction in pulmonary hypertension

Rajagopalan N et al. – Pts with pulmonary hypertension (PH) show abnormal left ventricular (LV) function quantified by elevated LV myocardial performance index (MPI), which correlates significantly with pulmonary vascular resistance and cardiac index.
Methods- Study to quantify LV function in PH pts by measuring LV MPI
- Correlation with invasively determined hemodynamic variables
- Prospective measurement of LV MPI via transthoracic echocardiography
- Subjects: 50 pts with PH (53±11 yrs; 35 women) who had right heart catheterization within 1 day of echocardiography and 15 healthy controls
- LV MPI significantly increased in PH group vs controls, indicating worse LV dysfunction
- LV ejection fraction did not differ significantly between groups
- LV MPI showed significant correlations with invasively determined mean pulmonary artery pressure, pulmonary vascular resistance, and cardiac index
- By receiver operating characteristic analysis, LV MPI >0.75 predicted cardiac index <2 L/min/m2 with 89% sensitivity and 78% specificity
- In multivariate model, LV MPI independently associated with cardiac index
