Identifying left ventricular dysfunction in pulmonary hypertension

Rajagopalan N et al. – Pts with pulmonary hypertension (PH) show abnormal left ventricular (LV) function quantified by elevated LV myocardial performance index (MPI), which correlates significantly with pulmonary vascular resistance and cardiac index.

Methods

Study to quantify LV function in PH pts by measuring LV MPI
Correlation with invasively determined hemodynamic variables
Prospective measurement of LV MPI via transthoracic echocardiography
Subjects: 50 pts with PH (53±11 yrs; 35 women) who had right heart catheterization within 1 day of echocardiography and 15 healthy controls

Results

LV MPI significantly increased in PH group vs controls, indicating worse LV dysfunction
LV ejection fraction did not differ significantly between groups
LV MPI showed significant correlations with invasively determined mean pulmonary artery pressure, pulmonary vascular resistance, and cardiac index
By receiver operating characteristic analysis, LV MPI >0.75 predicted cardiac index <2 L/min/m2 with 89% sensitivity and 78% specificity
In multivariate model, LV MPI independently associated with cardiac index
