Weiss GG – With the rapid and sustained growth of the hospitalist movement, cooperation between teams of physicians becomes all the more important. Sharing information on patients' charts and discussing treatment and discharge strategies make for easier patient transitions to and from the hospital and promotes continuity of care during and following hospitalization.
