Bingham D – Obstetric hemorrhage is an emergency situation in which clinicians can make errors that cause women to suffer preventable maternal morbidity and mortality. Scrutinizing commonly occurring obstetric hemorrhage–related practice errors by applying the generic errors modeling system, a research–based framework, to quality improvement efforts facilitates the identification of error specific reduction strategies.
