Intrajejunal levodopa infusion in Parkinson's disease: A pilot multicenter study of effects on nonmotor symptoms and quality of life

Honig H et al. – Results are the first findings that a levodopa-based continuous dopaminergic stimulation is beneficial for nonmotor symptoms (NMSs) and health-related quality of life in Parkinson's disease (PD) in addition to reduction of motor fluctuations and dyskinesias.
Methods- Prospective open-label observational study of the effects of intrajejunal levodopa/carbidopa gel infusion on NMS in PD pts followed for 6 mo
- Standard assessments by Nonmotor Symptoms Scale (NMSS) and Unified Parkinson's Disease Rating Scale (UPDRS 3 motor and 4 complications)
- Quality of life (QoL) assessments by Parkinson's disease questionnaire (PDQ-8)
- Subjects: 22 advanced PD pts (mean age 58.6 yrs, disease duration 15.3 yrs)
- Statistically significant beneficial effect in 6 of 9 domains of NMSS: cardiovascular, sleep/fatigue, attention/memory, gastrointestinal, urinary, and miscellaneous (including pain and dribbling)
- Statistically significant benefits for total score of this scale (NMSST) paralleling improvement of motor symptoms (UPDRS 3 motor and 4 complications in best on state) and dyskinesias/motor fluctuations
- Significant improvements using Parkinson's disease sleep scale (PDSS) and PDQ-8 (QoL)
- Improvement in PDQ-8 scores correlated highly significantly with changes in NMSST
- Moderately strong correlation with UPDRS changes
