Polyzos SA et al. – These data suggest that serum HCY levels are increased in patients with PDB. A single ZOL infusion results in a decrease in HCY levels that might represent another mechanism for the reduction of the activity of PDB achieved by ZOL.

This is the first report of serum homocysteine (HCY), folate and vitamin B12 in patients with Paget's disease of bone (PDB). There are limited data regarding the effect of bisphosphonates on HCY, folate or vitamin B12. In this study, higher serum HCY and folate levels were observed in the pagetic compared with the control group and HCY levels were decreased 3 months after zoledronic acid (ZOL) infusion and remained essentially unchanged up to the end of the study. Since the increased HCY could lead to increased activation and formation of osteoclasts, this effect of ZOL on HCY might represent another mechanism for the reduction of the activity of PDB achieved by ZOL.
