Implementing the Chronic Care Model: A Statewide Focus on Improving Diabetes Care for Pennsylvania
Clinical Diabetes, 10/20/09
Siminerio L et al. – In a state in which much of the best of American medicine was born and flourished, the implementation of a chronic care program has the potential to reenergize practitioners and patients, who will be given the resources necessary to manage their own well–being. Lessons learned from this initiative ultimately have the potential to serve as a model for other states in ensuring a consistently high quality of care for their citizens.
