Hu S et al. – A pulmonary arteriovenous malformation (PAVM) is a rare, congenital vascular malformation. Historically, treatment is embolization with coils or a detachable balloon to prevent the risk of serious complications, such as cerebral embolism/infarct, brain abscess, paradoxical emboli, and high–output right–heart failure. In this patient case, lifestylelimiting symptoms, including dyspnea on exertion and epistaxis, were successfully treated with transcatheter embolization of a large PAVM using a recently developed device called a vascular plug...The vascular plug appears to be an effective tool for embolotherapy of simple PAVMs, particularly in cases with a single large feeding and draining vessel. These patients are at an increased risk of inadvertent embolization when coils or detachable occlusion balloons are used. Long–term results are not yet available; however, our patient will be closely followed with serial CT by the pulmonology service for any indications of recanalization of the PAVM. Screening was recommended for her family members.
