Teriparatide superior to alendronate for glucocorticoid-induced osteoporosis

Pucino F – Study reports that teriparatide (a recombinant human parathyroid hormone) is more effective in increasing lumbar spine bone mineral density than alendronate (Fosamax—Merck) for patients suffering from glucocorticoid (GC)-induced osteoporosis.
Methods- Multicenter, randomized, trial evaluates the efficacy and safety of teriparatide vs alendronate in pts with GC-induced osteoporosis
- 427 ambulatory pts (≥21 yrs): 277 postmenopausal women, 67 premenopausal women, and 83 men
- Before screening, all pts had received 5 mg/d or more of prednisone for 3 or more mo
- Pts were randomized to receive:
- Teriparatide (n=213)- 20 mcg/d S.C. plus placebo tablets administered orally
- Alendronate (n=214)- 10 mg/d orally plus placebo sc
- In addition, all pts received supplemental calcium (1,000 mg/d) and vit D (800 IU/d)
- Primary outcome: changes in lumbar spine BMD
- Secondary outcome: changes in hip BMD, bone biomarkers, the incidence of fractures, and safety
- Results from the 18-month primary phase of this 36-mo study:
- Mean % increases from baseline in lumbar spine BMD were greater for the teriparatide groups
- Radiographic vertebral fractures occurred in 1 patient randomized to teriparatide and 10 pts randomized to alendronate
- Nonvertebral fractures occurred in 12 pts randomized to teriparatide and 8 pts randomized to alendronate
- Prportion of pts reporting serious AEs was comparable between the 2 treatment groups
- However, nausea was reported by more pts receiving teriparatide
