Kiratli H et al. – Leukaemic infiltration of extraocular muscles is a rare and late manifestation of the advanced disease associated with relapse and there seems to be a predilection for the lateral rectus muscle. Systemic prognosis remains dismal despite intensive chemotherapy.

Even a busy ophthalmologist rarely sees a patient with leukemic ocular involvement in daily practice. This is partly due to the fact that ophthalmic involvement is indeed unusual on one hand, and when leukemic infiltrates are evident in the eye and orbit, the patient is usually at the terminal stage so that ophthalmological consultation never becomes a priority on the other hand. Isolated extraocular muscle involvement is perhaps one of the most uncommon manifestation of ocular leukemia. This article summarizes the clinical and radiological features of extraocular muscle leukemia which may easily masquarade as inflammatroy conditions.
