Distinctive retinal nerve fiber layer and vascular changes in neuromyelitis optica

Green AJ et al. – Nerve fiber layer (NFL) and fundoscopic findings suggest that some of injury in neuromyelitis optica (NMO) may be vascularly mediated. These inner retinal vascular changes may share a common pathology to optic nerve and spinal cord (SC) autopsy findings, suggesting that vascular changes may be detectable during life.
Methods- Cross-sectional study comparing retinal features of optic neuritis (ON)
- Subjects: 20 multiple sclerosis (MS) vs 16 NMO pts with a history of ON (visual acuity at time of attack > 20/100) matched for age and gender
- Use of optical coherence tomography (OCT) and fundoscopy
- Vascular changes: attenuation of peripapillary vascular tree (3/40 MS eyes; 22/32 NMO eyes); focal arteriolar narrowing (0/40 MS eyes; 9/32 NMO eyes)
- Lower average NFL thickness for NMO vs MS pts (59.2 microm vs 82.0 microm)
- After controlling for final visual acuity, ~twice NFL thinning for NMO vs MS pts (32.1 vs 17.6 microm)
- More severe and diffuse axonal injury of the NFL for NMO vs MS pts
