The role of prepregnancy maternal physiology in both the normal and abnormal outcomes of pregnancy has been a focus for our research efforts. In this study we evaluated volumetric uterine blood flow and cardiac output in addition to the more frequently measured Doppler acquired wavefrom indices. The study is small, resulting from the challenges associated with acquiring data prior to a first pregnancy and then longitudinally through pregnancy, so the failure to find significant correlations in some comparisons may well suffer from inadequate power to detect real differences. Nevertheless we do find significant association in some hemodynamic measures between the prepregnancy and early pregnancy state as well as some interesting trends suggesting reduced uterine blood flow and lower cardiac output in prepregnancy physiologic status in women who went on to develop clinical preeclampsia in their first pregnancies.
