Our results suggest maternal caffeine consumption, even at low levels, is associated with modestly increased risk for neural tube defects. Recent studies of other birth defects also show slightly to moderately increased risk associated with maternal caffeine intake, but with no dose-response observed, it is difficult to rule out the effects of recall bias. If confirmed by other studies, the effect in our study appears large enough to warrant public health concern given the high and rising prevalence of caffeine exposure. Further studies are needed to determine whether current recommendations to reduce caffeine intake during pregnancy based on higher risk of other reproductive outcomes should be expanded to include avoidance of caffeine very early in pregnancy or even while trying to conceive.
