World Health Agency (WHO)
Reducing accidents and the risk of error in health care requires a significant and sustained response at global and national levels, across all levels of the health care system. For this reason, WHO and its partners launched the World Alliance for Patient Safety, in October 2004, to advance the patient safety goal "First do no harm" and cut the number of illness, injuries and death suffered by patients.

US Pharmacopoeia (USP)
