Possible Increase in HIV and Syphilis Prevalence Among Men Who Have Sex with Men in Guangzhou, China: Results from a Respondent-Driven Sampling Survey

MSM has become the third group of population most affected by HIV epidemic, right after injecting drug users in early 1990s and unsafe plasma donors in mid-1990s. In 2005, MSM transmission only accounted for 0.4% of the country’s HIV cases, whereas in 2007, the proportion increased to 3.3%. Of the estimated 50,000 new HIV infections in 2007, 12.2% were thought to be MSM transmissions. Guangzhou, as a metropolis of China, is also experiencing the rising HIV prevalence among MSM. In this respondent-driven sampling study, HIV prevalence in 2008 is estimated at 5.2% among MSM in Guangzhou, which is greatly higher than 0 in 2004 and 1.3% in 2006. the results of this study suggest a possible rapid raise of HIV and current syphilis infections among MSM in Guangzhou. Unprotected anal sex and current syphilis infection are associated with HIV infection. Targeted interventions with VCT and STI services are needed to address the epidemic with a focus on such subgroups as those with current syphilis, non-official Guangzhou residence status, low educational level, and who are married or divorced. This study contributes to the overall scientific evidence body in understanding the magnitude and trend of HIV epidemic among MSM, defining subgroups for more targeted intervention as well as to the application of respondent driven sampling method in recruiting hard to reach high risk groups including MSM.
