hypertension is now an established risk factor for dementia but therapeutic trials concerning the effect of antihypertensive therapy on cognition have produced controversial findings. We conducted an longitudinal study including 321 elderly Alzheimer's disease patients (mean age: 78 years) followed up 34 months. Means MMSE over time were significnatly higher in the group under antihypertensive treatment. Furthermore, in a multivariate analysis, after adjusting for confounding factors, the use of antihypertensive therapy was associated with a lower estimated risk of cognitive decline (as defined by a decrease of at least one point in MMSE score over time). Nevertheless we have to report that blood pressure measurement was not availabale at each visit so we aren't able to know if our result is due to blood pressrue drop or to another mechanism.Then, it is recommended that randomized studies comparing the effect on cognition of different antihypertensive therapy strategies, be considered for further investigations.
