Parkinson's disease dementia -- A diminished role for the Lewy body

Libow LS et al. – Findings do not support the hypothesis that Lewy bodies (LBs) are the main substrate for dementia in of Parkinson's disease (PD). More research with a larger sample size must determine whether the LB may be a secondary phenomenon and/or an “innocent-bystander”, as the entire role of LB in PD dementia is undetermined.
Methods- Study to characterize pathological features of PDD in 18 PD brain specimens by published criteria for AD, diffuse LB disease (DLBD), and vascular disease as a framework
- Among 16 PD dementia subjects, 3 (19%) did not have LBs outside of the brain stem and did not have AD or vascular pathology
- Of 2 additional cases that did not meet DLBD diagnostic criteria, 1 pt had rare LBs in neocortex and cingulate gyrus
- Remaining PD dementia subjects had a classic pathological profile consistent with AD (38%), vascular disease (12.5%), DLBD (6%), or a combination of these pathologies (12.5%)
