Bay E et al. – A review of the current treatment options for depression after mild traumatic brain injury (MTBI) summarizes the difficulties in diagnosis, the natural history, and the results of randomized and nonrandomized treatment trials.
Methods- Review of current treatment for post-MTBI depression
- Most common neurologic condition in United States; ~ 503 of 100,000 emergency department pts
- Complex cluster of neurologic signs and symptoms associated with acceleration-deceleration mechanism of injury
- Diagnosis/treatment complicated by post-MTBI depression and symptom overlap: delays in information processing, sleep difficulties, irritability, fatigue, headache, musculoskeletal pain
- Post-MTBI depression associated with abnormal CT scan results, older age, and increased depressive symptoms within 1 week
- Limited evidence-based treatment of post-MTBI depression
- Early interventions focus on managing pain and stress, with psychoeducational therapies
