Redistribution of slow wave activity of sleep during pharmacological treatment of depression with paroxetine but not with nefazodone

Argyropoulos SV et al. – Slow wave activity (SWA) and rapid eye movement (REM) during antidepressant treatment appear interdependent; neither alone is likely to predict response to treatment. Higher modified delta sleep ratio (mDSR) did not predict therapeutic response. Serotonin (5-HT[2A]) blockade by nefazodone does not increase SWA above normal levels.
Methods- Study of the index DSR and its relationship to REM suppression before/during long-term treatment with nefazodone (no REM suppression) and paroxetine (REM suppression)
- Study of effect of 5-HT(2A) receptor blockade on evolution of SWA during treatment
- Double-blind, randomized, parallel group, 8-wk study in 29 depressed pts
- Sleep electroencephalograms: at home at baseline, on nights 3 and 10, and at 8 wks of either paroxetine or nefazodone treatment
- SWA automatically analyzed
- mDSR derived as ratio of SWA amount in 1st 90 min of sleep to that in 2nd + 3rd 90-min periods
- At baseline, similar SWA nighttime pattern to other reports of depressed pts
- mDSR improved over treatment course
- No difference between remitters and non-remitters
- Significant drug effect and a significant drug x time effect
- Much higher post-treatment mDSR for paroxetine pts, regardless of clinical status
