A randomized prospective long-term study of two oral appliances for sleep apnoea treatment

Ghazal A et al. – Findings show that both the IST and the Thornton Anterior Positioner (TAP) appliances are effective therapeutic devices for obstructive sleep apnea (OSA) after a period of >24?mo. Lack of compliance may be due to insufficient improvement in anticipated subjective symptoms and/or a recurrence of symptoms over time.
Methods- Comparison of long-term effect of 2 different appliances in OSA treatment
- Randomization of 103 OSA pts to an IST or TAP appliance
- Followed-up after short- (6?mo) and long-term (24?mo) treatment periods
- Sleep studies in sleep laboratory with/without appliances
- Questionnaires for subjective daytime sleepiness, sleep quality, quality of life and symptom scores
- Significant improvement in short-term with both appliances for quality of life, sleep quality, sleepiness, symptoms and sleep outcome
- Significantly greater effect with TAP appliance
- After >2 yrs of treatment, equal effect for sleep outcomes with both appliances
- Subjective benefits initially achieved lessened significantly
