Clinical assessment of topiramate therapy in patients with migrainous vertigo

Gode S et al. – Overall topiramate was effective in reducing the frequency and severity of vertigo and headache attacks. Both drug doses were equally efficacious. The 50-mg/day dose may be preferable as higher adverse effects were noted with the 100-mg/day dose.
Methods- Study of topiramate efficacy to reduce both vertigo frequency and severity and headache attacks in pts with migrainous vertigo
- Comparison of 50-mg/day and 100-mg/day doses
- Assessment of 30 pts diagnosed with definite migrainous vertigo
- Determination of vertigo and headache frequency as monthly number of attacks
- Determination of severity by visual analog scales measured 0 to 100 mm
- Randomization of pts to either 50 or 100 mg/day topiramate for 6 mo
- Evaluation of vertigo and headache frequency and severity at the end of study period
- Number of monthly vertigo attacks and monthly headache attacks decreased significantly in overall group after treatment
- Statically significant improvement in vertigo severity
- Headache severity showed significant improvement
- No statistically significant difference between high- and low-dose groups regarding efficacy
- Treatment discontinued due to adverse effects in 4 pts in high-dose group at end of first month
