The significance of oligoclonal bands in multiple sclerosis: Relevance of demographic and clinical features, and immunogenetic backgrounds

Idiman E et al. – Results suggested that, MS pts with CSF oligoclonal IgG bands (OCBs) have a female predominance, a better clinical course with less disability, and a better prognosis, are associated with HLA-DR15.
Methods- Examination of correlation between the frequency of OCB and clinical and demographic features of MS in Turkish MS population
- Determination of whether, population with OCB-positive and -negative MS constitutes distinct subpopulations in terms of clinical, demographic, and genetic base
- Assessment of 210 clinically definite MS pts
- Pts assessed clinically at baseline, 1 mo later, every 3 mo, then at year 5
- No CSF OCB in 30 (14.3%) cases
- OCB-positive: 141 of 156 female pts (90.4%); 39 of 54 male pts (72.2%)
- Female to male ratio higher in OCB-positive vs -negative group
- Clinical course and disability similar in 2 pt groups
- Mean relapse severity higher in pts without OCBs in CSF
- EDSS values worsened in OCB-negative group in year 5
- EDSS significantly higher in OCB-negative group in year 5
- No statistically significant difference for use of disease modifying therapy between the 2 groups
- HLA DR15 antigen frequency statistically higher in OCB-positive vs -negative pts and control group
