Quantitative evaluation of balance in patients with spinocerebellar ataxia type 1: A case control study

Mohan G et al. – Pts with spinocerebellar ataxia type 1 (SCA1) had global impairment of balance, with greater instability in anterior–posterior than medio–lateral directions. Apart from illness severity and duration, body weight was detriment to balance maintenance of in SCA1.
Methods- Quantitative assessment of balance in SCA1
- Subjects: 20 pts (males: 14, females: 6) with genetically positive SCA1 and 20 age- and gender-matched healthy subjects
- Ataxia rated by International Cooperative Ataxia Rating Scale (ICARS)
- Balance assessed by dynamic posturography for 2 key points
- 1) ability to control balance in all directions (overall balance index, OBI), front to back (anterior–posterior index, API) and side-to-side (medio–lateral index, MLI)
- 2) limits of stability (LOS) in all directions
- Balance index defined as abnormal if actual value exceeded predictive value
- Impaired balance in 80% of pts: all indices in 35%, OBI + API in 25%, only OBI in 15%, OBI + MLI in 5%
- Significantly higher balance indices and lower LOS scores for SCA1 pts vs controls
- Significantly higher mean value of API than MLI value for SCA1 pts vs controls
- LOS was best predictor of balance abnormality
- In SCA1 pts, significant positive correlations for all balance indices with ICARS, static score of ICARS, body weight, severity and duration of illness, but not with the CAG repeat length
