About the role of visual field defects in pure alexia

Pflugshaupt T et al. – A list of 3 criteria is proposed that may improve the differential diagnosis of pure alexia and enable appropriate therapy recommendation.
Methods- Study comparing eye movement patterns during text reading in 6 pts with pure alexia vs 6 pts with hemianopic dyslexia showing similar right-sided visual field defects
- Role of field defect in reading difficulties of pure alexics was highly deficit-specific
- Exclusion of lesions of hemianopic dyslexic pts vs pure alexic pts showed largest group differences in posterior parts of the left fusiform gyrus, occipito-temporal sulcus and inferior temporal gyrus
- These regions included the coordinate assigned to the center of the visual word form area in healthy adults, supporting a relation between pure alexia and a damaged visual word form area
